Anti-Patterns
- Orphan wiki dumps.
- Uncited factual claims.
- Duplicate entities.
- Appending stale layers to compiled truth.
- Using tools without routing.
- Calling work done without proof.
- One-off repeated workflows.
- Sycophancy instead of judgment.
- Unrelated refactors.
- Abandoned processes or hidden dirty state.
Correction Rule
Fix the mechanism, not the symptom. If an anti-pattern appears twice, update a skill, protocol, doctor, or automation.
Failure Mode
The most dangerous bad output is confident and convenient: it saves a minute now by making future sessions reconstruct state.
